Workplace Investigations: Uncovering Misconduct & Omissions

Workplace Investigations: Uncovering Misconduct & Omissions

Workplace investigations require impartiality, thoroughness, and compliance with the Occupational Health and Safety Amendment Act (Violence and Harassment in the Workplace) 2009. An external investigator brings credibility and avoids bias, while reducing risk and cost.

We investigate suspected misconduct, misleading statements, and material omissions — especially in workplace injury claims where false medical or financial representations can have serious repercussions for liability, compensation, and insurance premiums. We provide detailed written reports and actionable evidence.

Our scope includes internal compliance and corporate risk, workplace misconduct, workplace safety violations, theft and fraud, harassment and discrimination, sexual harassment investigations, whistleblowing and ethical violations, competitive intelligence, due diligence for mergers and acquisitions, and human resources investigations. We can also investigate the spouse, dependants, and medical practitioners involved in a claim.

Our Background Check & Credential Verification Process

Background checks on individuals and corporate entities are essential for establishing credentials. We perform in-depth background checks on prospective and current employees, executives, business partners, and corporate entities (private and public).

  • Verify identity and address
  • Prior criminal record check
  • Character check
  • Bankruptcy record review
  • Credit history analysis
  • Asset check
  • Pending legal proceedings, lawsuits and judgments
  • Employment record and education verification

Insurance Fraud Investigations FAQs

How long does an insurance investigation typically take?

Most investigations run 1–4 weeks, depending on the claim complexity and the subject's patterns. Staged-accident cases may be resolved in a single surveillance shift; long-term disability claims may require multiple operations over a month to document routine and contradictory behavior. We give you an honest timeline at the start and stop when you have what you need.

Is surveillance evidence admissible in court?

Yes, when collected by a licensed PI following Ontario law. Our investigators are licensed under the PSISA, use court-admissible equipment, and produce signed reports with timestamped video and photo evidence that have held up in insurance claims and civil court proceedings.

What if the investigation finds nothing suspicious?

That's a valid outcome. Sometimes claims are legitimate, and we report that clearly. You avoid pursuing a dubious claim further, and you have documented proof of due diligence if the claim is contested later.

Can you investigate a workers' compensation claim?

Yes. Workplace injury claims are among the most commonly investigated. We document whether the claimant is truly incapacitated, whether material facts were omitted (medical or financial), and whether misconduct is involved — all within the law and OHSA requirements.

What is insurance fraud surveillance?

Surveillance conducted to verify the legitimacy of an insurance claim — usually disability, accident-benefits, or workers' compensation. We document the claimant's actual physical capacity, daily routine, and activity level, then compare it against what they've reported to the insurer or the Tribunal. Any contradictions are evidenced with timestamped video.

How do you prove a staged auto accident?

Through pre-loss and post-loss surveillance, vehicle history analysis, claimant network mapping (looking for repeat passengers across unrelated claims), social-media investigation, and locating witnesses. Staged-accident rings have signatures — same passengers, same clinics, same lawyers, similar collision profiles — that systematic investigation can expose.

What is a "day-in-the-life" investigation?

A multi-day surveillance operation that documents a claimant's full activity pattern — driving, lifting, walking distance, social activity, work — to compare against the limitations they've reported. Day-in-the-life evidence is highly persuasive at LAT hearings, accident-benefits disputes, and long-term disability tribunals.

Can investigation evidence be used at the Licence Appeal Tribunal (LAT)?

Yes. LAT routinely admits surveillance video, investigator reports, and signed witness statements when properly collected and disclosed. We deliver evidence packages formatted for LAT submission and our investigators are available to testify if the adjudicator requires.

Who hires insurance investigators — insurers, lawyers, or claimants?

All three. SIU departments, defence counsel, and adjusters retain us to investigate suspect claims. Plaintiff lawyers retain us for liability and damages investigations. We do not work both sides of the same matter, and we maintain confidentiality strictly within whichever side has retained us.

Can you investigate property damage, fire, or theft claims?

Yes. Property-loss investigations involve scene analysis, witness interviews, financial-motive review, prior-claim history, and where appropriate, undercover or pretext interviews to identify staged losses, arson-for-profit, or inflated claims. Findings are reported with photographic, documentary, and statement evidence.

What if the claimant lives in another province or country?

We coordinate with vetted licensed investigators in other Canadian provinces and internationally. The case is project-managed from our office so reporting, chain of custody, and quality control stay consistent — you deal with one point of contact regardless of where the surveillance happens.
